Fill in all blank fields before signing. In Wyoming, using a non-statutory form may void the waiver. This form must be notarized.
Wyoming statute § 29-10-101 specifically requires notarization for lien waivers to be valid. This is unusual — only Wyoming and Mississippi have this requirement. A non-notarized lien waiver in Wyoming is unenforceable.
