Fill in all blank fields before signing. In Utah, using a non-statutory form may void the waiver. No notarization required.
Subcontractors and suppliers must serve a preliminary notice within 20 days of first furnishing. Without it, lien rights are severely limited. The notice must be sent to the owner, the general contractor, and any construction lender.
