Fill in all blank fields before signing. In Texas, using a non-statutory form may void the waiver. No notarization required.
GCs commonly condition final payment on receipt of unconditional waivers from all subs. This is standard practice — but make sure the payment is actually in your account before signing. Many contractors sign conditional final waivers with pay applications and exchange unconditional finals simultaneously with the final check.
